Vive Tu Vida En Forma is a non-for-profit organization focused on health and wellness for communities of color which aims to improve the overall health and well-being of individuals from racial and ethnic minority communities.  The Vive Tu Vida En Forma organization provides targeted programs and services that address the unique health needs and challenges of communities of color.  This includes health education and outreach, disease prevention, and management programs, mental health support, and access to affordable healthcare services.
